Please fill out above form to request your prescription refill or food item ,this will save you time when picking up your order.

New prescriptions require your pet to be examined and some of repeat prescriptions will also require your pet to be examined before refilling ......to avoid any side effects as well as to determine that your pet is getting adequate dosage...we will let you notify before pick up..

Please allow 24 hours for order processing ,please do not come to the clinic until you have recieved a confirmation to pick up order.
